Interventional psychiatry
Spravato and TMS need more than a claim
Spravato (esketamine) can only be given in healthcare settings certified under the Spravato REMS program, with patients monitored after each dose. Payers usually require prior authorization, and they differ in how the drug and the observation time are billed. For Medicare, CMS created specific HCPCS codes for esketamine treatment visits; many commercial payers use their own rules.
TMS is usually authorized as a course of treatment with a set number of sessions. Payers often require documentation of failed medication trials and a standardized depression scale. We check these requirements before the first session, not after the first denial.
