What CAQH is
CAQH ProView is the profile most commercial payers use to credential you
CAQH ProView is an online database where you enter your credentialing information once. Payers you authorize pull your data from it instead of sending you a separate form.
Most commercial health plans and many Medicaid managed care plans use CAQH. Medicare does not; it uses PECOS. Most state Medicaid programs also use their own enrollment portals.
A CAQH profile does not enroll you anywhere by itself. Each payer still has to credential you and issue a contract with an effective date. But if your profile is incomplete, out of date or not attested, most commercial applications stop.

Register or recover access
We register a new profile or help you regain access to an existing one, and note your CAQH Provider ID.

Complete every section
Personal details, licenses, education, specialties, practice locations, hospital affiliations, malpractice coverage and work history, entered to match your license and NPPES record.
- 03
Upload documents
We upload current copies of each required document and record expiration dates so we know when to replace them.
- 04
Authorize payers
We give the payers you are applying to permission to view your profile. Without this, a complete profile does nothing.
- 05
Attest
You review the profile and attest that it is accurate. We walk you through it. The profile is then available to authorized payers.
- 06
Re-attest every 120 days
CAQH asks you to re-attest every 120 days. We track the date, update anything that changed and remind you so you re-attest on time.
Documents
Documents your CAQH profile needs
Have these ready before you start. Missing documents are the most common reason a profile sits unfinished.
- State license for every state where you practice
- DEA registration and any state controlled substance registration, if you prescribe
- Malpractice insurance face sheet or certificate with current dates
- CV with month and year for every position, and an explanation of any gaps
- Board certification, if you have one
- Diploma or education and training details
- Practice locations, phone numbers and hours
- NPI numbers (Type 1 and, for groups, Type 2)
- IRS W-9 for the entity that will be paid

Common errors
CAQH errors that stall credentialing
Most CAQH problems are small mismatches or missed dates. Each one can hold up an application for weeks.

CAQH help is included with our credentialing
When we credential you, CAQH setup and re-attestation are part of the work. Credentialing starts at $80 per payer, and 5 commercial payers are credentialed free with our billing service.

Common questions
Is CAQH free for providers?
Yes. CAQH ProView is free for providers to use. Health plans fund it. You pay only if you hire someone, like us, to manage the profile for you.
How often do I need to re-attest CAQH?
Every 120 days. If you miss the date, payers cannot use your profile until you attest again, which can stall new applications and re-credentialing.
Does Medicare use CAQH?
No. Medicare enrollment goes through PECOS. Most state Medicaid programs use their own portals too, although some Medicaid managed care plans use CAQH.
Why can’t my payer see my CAQH profile?
Usually because the payer is not authorized to view it, the profile is not attested, or the attestation has lapsed. We check all three and fix the one that is blocking you.
Does completing CAQH mean I am credentialed?
No. CAQH only stores your information. Each payer still has to credential you and issue a contract with an effective date before you are in-network.
Can you take over an existing CAQH profile?
Yes. With your permission, we review the existing profile, fix errors and outdated documents, confirm payer authorizations and walk you through re-attestation. Then we track the next 120-day date for you.
